SHELBYVILLE, Ind. (AP) -- Indiana regulators have fined a central Indiana glass factory $150,000 after inspections found repeat violations of safety lapses discovered at the plant following a worker's 2010 death.
The Indianapolis Business Journal reports that Pilkington North America was fined following a spring inspection by the Indiana Occupational Safety and Health Administration at the Shelbyville plant about 25 miles southeast of Indianapolis.
A July 27 safety order alleges six "repeat" violations for the Toledo, Ohio-based subsidiary of Japanese conglomerate NSG Group.
Indiana Department of Labor spokesman Bob Dittmer says the agency has launched another investigation into safety violations at Pilkington's Shelbyville plant. He says agency officials "really, really don't like repeating violations."
In September 2010, 56-year-old Pilkington worker Kelly Dean Caudill of Connersville was fatally crushed in plant machinery.
