LOUISVILLE, Ky. (WDRB) - Police in Louisville have arrested a Bardstown woman after authorities say she unlawfully accessed a blank prescription pad and forged prescriptions for herself, and two others.

Elisha Shultz, 37, was arrested Monday morning for allegedly forging prescriptions for Hydrocodone and Oxycodone for herself and two others. Shultz allegedly stole a prescription pad while she was employed at a pain management clinic in Louisville.

Police say Shultz admitted that she had provided written prescriptions to the other individuals, who in turn filled them. Shultz also admitted to authorities that she had sold a full blank prescription pad for $150 and individual pills for $3 each.

Shultz allegedly filled at least four unauthorized prescriptions in her name at a Walgreens pharmacy, police say.

Shultz is charged with unlawful possession of a blank prescription, forgery of a prescription and prohibited acts related to controlled substances.
