LOUISVILLE, Ky. (WDRB) -- Residents of a Louisville nursing home are being relocated after the Centers for Medicare and Medicaid Services found reason to terminate its provider agreement with the facility after the inspector general found "severe problems."

Hurstbourne Care Centre houses 125 residents, but after several complaints and investigations by state agencies, those residents are being relocated, leaving the fate of the facility unknown. 

The termination was ordered Friday. A report more than 700 pages long outlines deficiencies state investigators found at the facility, including some testimony from families about a resident who woke up with ants all over the bed. 

We spoke to a former resident's wife who didn't want to go on camera but told us her husband went two months without a shower while he lived at Hurstbourne Care Centre. 

She said his caretakers neglected him. 

A statement from the inspector general says in part, "Several of the alleged deficiencies were determined to be an immediate and severe threat to the residents of the facility." 

It goes on to say that, "Problems at the facility were determined to be so severe that CMS today issued a notice of termination of the provider agreement with the facility, which is effective July 25th." 

It says, "CMS will reimburse the facility for Medicaid and Medicare residents for up to thirty days following the termination date. It is not known at this time whether the facility will choose to remain open."

It's unknown how many employees, if any, will lose their jobs. Some of them tell us transferring to another facility isn't an option because there's only one other Consulate Health location in Louisville. 

An employee at Hurstbourne Care Centre referred us to Consulate Health Care, its parent company. The company sent this response:

We are aware of the issues that have been raised by CMS at our Hurstbourne Care Centre at Stony Brook.  Our top priority is ensuring that our patients and residents continue to get the best care available related to their individual needs.  We have and will continue to support our leadership team at Stony Brook as they address these issues in an immediate and prompt fashion, and we will work to ensure that our patients and residents do not experience any interruption of care.
