LOUISVILLE, Ky. (WDRB) -- Kentucky State Police are investigating after an inmate at a Kentucky prison died.

The person was incarcerated at the Kentucky State Reformatory in Oldham County.

KSP received a call of a hostage situation at the prison around 5 p.m. Tuesday. An inmate, David Carver, allegedly took a nurse hostage with an "improvised edged weapon."

Troopers negotiated with Carver got two hours, but when those negotiations fell apart, the reformatory's Correction Emergency Response Team responded and assumed control from KSP.

The CERT team continued negotiations with Carver, but when he made a move toward member of the team with his weapon, Capt. Patrick Hawkins fired a shot, hitting Carver.

He was taken to Baptist Health in La Grange on Tuesday night, where he was pronounced dead at 7:25 p.m.

An autopsy is scheduled for Wednesday.
