LOUISVILLE, Ky. (WDRB) -- Authorities have identified a woman killed Monday morning in a crash on westbound I-64 in Floyd County, Indiana.

According to the Floyd County Sheriff's Department, that woman has been identified as 34-year-old Patricia Colwell of Georgetown, Indiana.

The crash happened near the 114 mile marker Monday morning.

A Floyd County dispatcher says two vehicles were involved in the crash. A call about the crash was received at 9:13 a.m.

Police say a Hummer H2 was going eastbound on I-64 when, for an unknown reason, it crossed into the westbound lanes and hit a Ford Fusion. Officials say Colwell was killed instantly.

A 15-year-old passenger inside the Fusion was treated for non-life threatening injuries, according to police. Investigators say the driver of the Hummer was taken to the hospital for minor injuries.

Authorities say a third vehicle was damaged by debris from the crash, but no one inside the vehicle was hurt.

Ryan Houchen, a spokesperson for the Lafayette Township Fire Protection District, says two people trapped inside a vehicle were removed from a vehicle.

The investigation into the crash is ongoing.
