LOUISVILLE, Ky. (WDRB) -- Jefferson County Public Schools has sent a letter home to families asking parents to provide or decline consent for health services provided by the district.
JCPS said the new form is because of Senate Bill 150 that was passed by Kentucky lawmakers earlier this year.
If parents or guardians consent, students would be eligible to be treated by school nurses for health or mental health issues.
The new form also has an opt-out for services related to human sexuality, contraception and family planning.
School nurses are available at every JCPS school and some high schools have health clinics.
