LOUISVILLE, Ky. (WDRB) - An investigation into safety at General Motors takes an odd turn.

Employees documenting potential safety issues are banned from using 68 words, including safety, chaotic, problem, failure, and defect.

The list is part of a 2008 employee training document.

It's one of the more unusual twists in the auto giant's ongoing recall of more than 2.5 million cars linked to defective ignition switches.

The words were on company documents released Friday, the same day the government fined GM $35 million for delaying response to the problem.
