Officer Ben Sullivan is accused of violating the department's chase policy during the incident on July 6, 2021.

Lamont Washington, an investigator with the Louisville Metro Police Professional Standards Unit, said an officer did break the department's pursuit policy before a 2021 crash that killed 22-year-old Trevon Mitchell.

Trevon Mitchell, 22, was killed when a suspect blew through a stoplight and hit his moped in July 2021.

Police say they tried to stop 31-year-old Larry Williams — but he sped away, and now one person is dead.

According to Kentucky State Police, a crash involving a 2017 Chevrolet Corvette and a 2012 Mini Cooper occurred near mile marker 41 around 9:11 p.m.
