Humana Tower in downtown Louisville, Ky.

Humana, the only Fortune 500 company headquartered in Kentucky, plans to vacate its headquarters building at 500 W. Main Street to "centralize" its Louisville employment in the company’s Waterside-Clocktower campus on the east side of downtown.

The Louisville-based healthcare company was given until Sept. 24 to make the  payment, according to a court filing.
