LOUISVILLE, Ky. (WDRB) -- A man died while working at a Louisville Gas & Electric facility late Monday night, according to the Jefferson County coroner's office. 

Michael Sallengs, 41, was killed around 11:45 p.m. at the Mill Creek plant at 14660 Dixie Highway, the office said. 

The cause of death was listed as "injuries sustained in an industrial accident."

LG&E spokeswoman Natasha Collins confirmed the death of a contract worker in a statement on Tuesday:

"We can confirm that a fatality occurred at our Mill Creek facility Monday night. The fatality involved a male contractor from a company called The Wood Group who was working in a portion of the plant and was discovered just after 11 p.m. (EST). At this point we are working with police in their investigation and are also working with the Wood Group on an internal investigation into what happened.

Our hearts are with his family and loved ones, the Wood Group and all of the employees who worked with him. A loss like this is felt throughout the Mill Creek facility and across all of our LG&E and KU family and we have resources available to assist our employees."

The death is the second at Mill Creek in less than two years. Jaxxyn Lee Wood, a diver who was working for a contractor, drowned in July 2021 while conducting an inspection.

The Kentucky Labor Cabinet discovered nine serious violations and issued a $63,000 fine to LG&E in connection with that death. The utility has contested that penalty.  

LG&E also avoided a fine after a 2017 inspection at Mill Creek found four violations – three of them deemed serious. It later appealed an $18,900 penalty and ultimately settled the case without paying anything.
