LOUISVILLE, Ky. (WDRB) — With the flu, COVID-19 and respiratory virus season in full swing, Louisville's health department issued a reminder Friday for residents to check their vaccination history.
The Louisville Metro Department of Public Health and Wellness said the best place to start is with the health care provider or clinic that gave you the vaccine. If your provider uses MyChart, vaccination records can be viewed and downloaded there as well.
If you got your vaccines at a pharmacy, you can request your records from that location. You can also get your official immunization record, or your child's, through the Kentucky Immunization Registry through a public portal by clicking here.
The health department said insurance companies may be able to look at claims data to find out where a vaccine was administered, however insurance claims are not considered an official vaccine record.
When updating your vaccination status, the health department said individual situations matter. Factors such as age, vaccine type, health risks, and medical history may influence eligibility for certain vaccines.
The health department offers an Immunizations Clinic where you can get caught up on routine vaccines. It's available to those ages 2 months and older, including adults. For more information, click here. You can also call (502) 574-5380 to schedule an appointment.
Officials said those who visit the clinic should know their vaccination history. Records have some specific details that need to be included, such as patient demographic information, vaccine manufacturer, lot number, date of administration and the clinic site or a provider's signature.
